Supported Independent Living
Our Supported Independent Living (SIL) homes offer assistance with supervising of daily tasks and focus on developing the skills of each individual to live as independently as possible. We are proud to offer a variety of tailored SIL options matched to your individual circumstances and needs.
Our homes are staffed 24 hours a day, 7 days a week by a team of dedicated and trained Support Workers and a Registered Nurse.

To be eligible to receive these supports in our home, you need to have or be eligible for SIL funding approval within your NDIS plan. This is listed under the NDIS Core Supports funding as Category 1 – Daily Supported Living.
​
We provide all the necessary support for daily living such as assisting participants with cooking, cleaning, laundry, personal care and medication management based on the individual’s support needs.
Transportation assistance to day programs and appointments and community engagement activities can also be provided.
​
Short Term Accommodation (STA) Respite
We can provide Emergency Short Term Accommodation in your time of need. STA respite allows you or your loved one to have a short holiday and offers an opportunity to meet new people, make new friendships, explore new environments, and learn new skills. We work with you to plan your STA, ensuring that it’s right for you!
Our STA package covers meal costs and daily activities chosen by the client. Your goals are our goals, and taking time out when needed is such an important part of working towards a happier, more supported and independent life! You’ll be rest assured with the safety, security and high quality of care with our STA packages.
Medium Term Accommodation (MTA)
Medium term accommodation (MTA) is available to NDIS participants who require temporary transitional housing while they wait for their permanent home to become ready.
Typical examples of where MTA might be used include: participants who are ready to leave hospital but are waiting for disability related modifications to be completed in their home or participants awaiting SIL or SDA approval from the NDIS.

Community Participation
Participating in the community is vital for health and well-being. It promotes a sense of belonging, builds a network of social support and allows for opportunities to be physically active. So let's pursue your passions and learn new skills together!

At Access Home Care and Travel, we can support you one-on-one or in a small group. We always ensure you feel comfortable and tailor community programs based on your talent, interests and hobbies.


With the help from our support worker you can:

Explore your creative side through music, dance, drama or cooking programs.

Try new activities and experiences such as arts and craft, technology or fitness activities.

Make new friends through social activities, or in local clubs and community groups.

Go shopping, watch a movie, attend a TAFE or Community College class and much more!
Improved Daily Living & Travel Support
Our support workers can assist you with your day-to-day life such as developing skills to improve hygiene, get ready for the day, manage your life, and travel independently. This can include:

Showering, dressing and other personal care tasks

Household tasks including cleaning and cooking

Life skills including budgeting and meal planning

Travel and transport, including using public transport so that you are able to get to and from appointments, run errands, as well as participate in the community

Public Transport Travel Training
We travel with you as you learn a route and will do repeat practice sessions so you can travel safely and independently with confidence. The service is personalised and flexible, helping you deal with any challenges and choosing the best transport options for you.

Travel training includes:
- Catching trains, buses and ferries
- Using an Opal card
- How to plan trips
- Which service to catch
- When and where to get on and off
- Accessible services
- Strategies to deal with difficulties
- Dealing with emergencies
- Using transport apps


Community Nursing & Allied Health Care
- Community Nursing Care

Community Health Nurses work with people and their families to help prevent disease, maintain health and treat any existing health problems. Community nurses aim to support and maintain participants' independence, safety and a healthy lifestyle, as well as helping a participant’s carer learn the skills and support needed to live independently at home.
Our dedicated registered nurses can provide services including:

Post hospital care

General nursing care including assistance with chronic conditions and providing hospital type care outside of the hospital setting

Palliative care nursing assistance in conjunction with allied health professionals, for people who have a life-limiting illness and require support to stay at home

Wound management

Our Exercise Physiologists and Personal Trainers will support your physical well-being and help you achieve your goals.
